• a runny nose (not related to a pre-existing condition)
• Nasal congestion(not related to a pre-existing condition)
• Shortness of breath (not related to a pre-existing condition)

B. Have you travelled internationally within the last 14 days?

C. Have you had unprotected close contact with individuals who have a confirmed or presumptive diagnosis of COVID-19 (e.g. individuals exposed without appropriate PPE in use).

If YES to any of the above, you will not receive treatment in the clinic, please call Health Link 811.
